Verso, operated by Dr. Hormone MD (“we”), provides hormone therapy to adults in New Jersey, New York, Pennsylvania and Delaware. This page explains what we collect and how we use it. It also includes our HIPAA Notice of Privacy Practices, which explains how we protect your medical information and your rights over it.

What we collect

  • Contact details: name, email, phone, date of birth, state and address, including details you enter on our booking form even if you don’t finish.
  • Health information: your history, symptoms, labs, prescriptions and visit notes.
  • Payment details: processed by our payment processor.
  • Website data: pages visited, device, browser and IP address, collected through cookies and similar tools.

How we use it

  • To provide your care, prescribe, order labs and coordinate with pharmacies.
  • To schedule and remind you about appointments, labs and refills.
  • To follow up if you start but don’t finish a booking or purchase.
  • To send you news and offers about our own services by email (you can unsubscribe at any time) and by text if you opt in.
  • To bill you, run the practice, and meet legal requirements.
  • To improve our website and measure our advertising.

Who we share it with

  • Service providers who help us run the practice, such as our medical records, scheduling, messaging, payment and hosting providers. Those that handle health information sign agreements to protect it.
  • Pharmacies and labs that fill your prescriptions and process your tests.
  • Authorities when the law requires it.

We do not sell your health information.

Cookies and advertising

We use cookies, pixels, analytics and advertising tools (such as Google and Meta) on our website to understand how visitors use the site and to show our ads to people who may be interested. You can block or delete cookies in your browser settings.

Texts and emails

When you give us your phone number and email to book, you agree to receive messages about your booking and care, such as confirmations, reminders, and notices about lab results and refills. We may also email you about finishing a booking or about our services, and you can unsubscribe at any time. We send marketing texts only if you opt in. Reply STOP to stop texts and HELP for help. Message and data rates may apply. We do not share your mobile number or text opt-in with third parties for their marketing.

Security

We use reasonable safeguards to protect your information. If a breach affects you, we will notify you as required by law.

Children

Our services are for adults 18 and over. We don’t knowingly collect information from minors.

HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

How we use and share your health information

  • Treatment. To care for you and coordinate with others involved in your care. Example: sending your prescription to a pharmacy or ordering your bloodwork from a lab.
  • Payment. To bill and get paid for your care. Example: processing your payment for a consultation and labs.
  • Operations. To run the practice and improve care. Example: reviewing records for quality, or using a vendor to host our patient portal.
  • Reminders and our services. To remind you of appointments and tell you about our own treatments and services.

Other uses allowed or required by law. We may also share your information, without your authorization, when the law requires it, and for public health and safety reporting, prescription-monitoring programs, health oversight and licensing-board audits, lawsuits and legal proceedings, law enforcement requests, preventing a serious threat to health or safety, coroners and medical examiners, organ donation, research that meets legal privacy safeguards, workers’ compensation, and specialized government functions such as military and national security. If you are present, or in an emergency, we may share information with a family member or friend involved in your care unless you object.

Uses that need your written permission. We will not use or share your information for marketing by others, sell your information, or share psychotherapy notes without your written authorization. Any other use not described here also needs your authorization. You can revoke an authorization in writing at any time, except for actions already taken. Where state law gives extra protection (for example, for HIV, mental health or substance use information), we follow the stricter law.

Your rights

You have the right to:

  • See and get a copy of your medical and billing records, including electronically, usually within 30 days. We may charge a reasonable, cost-based fee.
  • Ask us to correct your records. If we say no, we’ll explain why in writing.
  • Ask us to contact you a certain way or at a certain address. We’ll say yes to reasonable requests.
  • Ask us to limit what we use or share. We don’t have to agree, except that if you pay in full out of pocket, you can ask us not to share that care with your health plan and we will agree.
  • Get a list of certain disclosures we made in the past six years.
  • Get a paper copy of this notice at any time.
  • Be notified if a breach affects your unsecured health information.

Our duties

We are required by law to protect the privacy of your health information, give you this notice, follow the notice currently in effect, and notify you of a breach. We may change this notice. Changes will apply to all information we hold, and the new notice will be posted on this page and available on request.
